Registration
Please make sure to register at the receptionist window when you enter our office. New patients making their first visit are requested to arrive 15 minutes before their scheduled appointment for the purpose of completing information Please be prepared to provide us with a copy of your insurance information. If your insurance card lists a Primary Care Physician, you will need to have a referral from this physician prior to seeing our physicians. You should check with your insurance company if you are unsure whether you need a referral. If a referral is required and not obtained, your insurance company will not allow or reimburse our charges.
Returning patients are asked to please provide us with any changes in address, phone number, insurance, employment, etc.
